SUCCESSFULLY NAVIGATING THE HOSPITAL / REHABILITATION DISCHARGE TRANSITION

As part of their ongoing speaker series, At Home on the Sound, a leading Westchester aging-in-place community, has partnered with STEPS Home Care and Honora Care Management & Consulting to host a 2 part presentation focusing on how to successfully navigate the hospital/rehab to home transition.
According to the Centers for Medicare & Medicaid Services, there is a 17.3% readmission rate within the first 30 days of discharge among Medicare recipients (65+). At Home on the Sound is a Center for Aging in Place village that serves the senior community in Mamaroneck, NY and Larchmont, NY. Therefore, to better prepare the members of their community they wanted to provide proactive solutions to the difficult transition back into the home after an acute incident.
The discussion will cover the entire process...starting with what to have on hand in preparation for a possible hospital visit...through a checklist of needs to safely return home. The more proactive you can be the better. It's very difficult logistically to gather all the necessary information AFTER one is hospitalized. So, if you, or a loved one, is susceptible to hospitalization it a good idea to get everything in order before an accident happens.
And it's also difficult to remember everything that should be discussed with a facility's discharge planner, if you don't have a comprehensive list of questions to have on hand. This list should include what equipment is needed to transition safely, what is the medication administration plan, what are the necessary services to which is one entitled, and for which services Medicare will pay.
Part 1 of the series covering proper preparation through admission will be on Tuesday March 12. Refreshments will be available at 3:30, and the presentation will begin at 4pm.
